Tubal Ligation Reversal by Dr. Levin

We are now offering a Fast-Track tubal reversal option for those of you who want to save both time and money getting on the surgery schedule. The elements of this approach are somewhat different than our regular program so I will give you an exact outline of how you should proceed. Potentially you could accomplish everything that needs to be done in about one hour! If you need questions answered with this program please email Linda or Erin at staff@babies-by-levin.com as they receive your sterilization records, medical histories and other documents before Dr. Levin. It's quicker for them to ask Dr. Levin about any issues you might have.

You must do: If you have not already done so then fill in the on-line initial contact form so we can be in touch with you. Please do not fill this form in again unless your address, phone number or e-mail address has changed.
You should consider doing: Download this software helper to your desktop to make the journey easier! Save it to your desktop.
You must do: Fill in our personal data and medical history form. Save the form to your “desktop”. After it downloads then click on the item on your “desktop” to open it. When you have filled in all four pages, re-save it to your desktop, attach it to an email and send it to me at history@babies-by-levin.com.
You must do: Download the medical records release form to your desktop. Fill in all of the blanks and then print it. Sign and date the release and then mail it to the hospital/clinic where you had your sterilization performed.
You should consider doing: There are a couple of lab issues that you may wish to consider. Women 40 years of age or over should have their blood drawn on cycle day 3 for FSH (follicle stimulating hormone) to check for “ovarian reserve”. This term relates to the ability of your ovaries to make and pass eggs that are capable of conceiving. You can contact Erin or Linda (below) for an order to do the FSH test and you should make sure to know that the result is normal before scheduling your reversal.

The second test is a sperm count to make sure your significant other is currently fertile. This can be done by having a sperm count done at a lab near you. Please contact Linda or Erin at staff@babies-by-levin.com to arrange either of these tests.
You must do: Check with Linda or Erin at staff@babies-by-levin.com to make sure we have received your medical records and your medical history form and that Dr. Levin thinks you are a candidate for the tubal reversal procedure.
You must do: Please read the 11 page section about tubal ligation reversal so that you understand what it's all about, what it costs and what your chances of success could be. By reading all of this you will avoid having to wait on a telephone consultation with Dr. Levin and move more quickly on to the actual reversal procedure.
You must do: Watch some of our videos to become better informed about how we can help you move toward having another baby!

You must do: Please enter your height and weight in the table below and then click on "Get Price". This will tell you the current fee for your height and weight and will also give you some idea of wheather you are close to the limit of costing more or costing less.

Once you find out how much the fee is, look at the bottom of the table to pay by credit card, apply for an online loan, start a 24 month prepayment plan or look at the various finance options that we have listed. You must do: Once you know we have the payment call 1-800-766-0304 to lock in your reversal date.
You must do: Any balance that you owe on your tubal reversal fee must be paid by wire transfer or by credit card in the table just above to us a minimun of two weeks prior to your reversal procedure. Make sure to enter your height and weight for the payment icons to appear in the table above. If you are over the height/weight group that you have paid for then any addition payment needs to be made at this time as well. If the balance of the fee is not paid by this time and in the fashion delinieated here, then the procedure will be cancelled and will not be rescheduled.
